KAISER Permanente’s COMMUNITY HEALTH

COVERAGE PROGRAM (CHCP)

(OPEN ENROLLMENT: NOV. 1, 2025 - JAN. 15, 2026)

What is Kaiser Permanente’s Community Health Coverage Program?

Kaiser Permanente’s Community Health Coverage Program  (formerly known as “Kaiser Bridge”) is designed to help those who are uninsured, income eligible, and actively enrolled with a participating community partner (i.e., a Georgia Organics Farmer Member) by providing help to pay for gold level health insurance through a Kaiser Permanente for Individual and Families (KPIF) plan.

How long does coverage last and what does it cover?

Once you apply, if you are accepted into the program, Kaiser will subsidize your monthly premiums (in full) from the time of enrollment until December 31 of the enrollment year. Families and children are applicable as well, and coverage includes preventive services, hospitalization, comprehensive pharmacy, and more.

A note from the Farmer Services team at Georgia Organics on farmer healthcare

We believe every farmer in Georgia deserves access to affordable, quality healthcare, regardless of where they live or work. Currently, the Kaiser Community Health Coverage Program is available only in counties where Kaiser operates. Despite our efforts, along with our partners at Kaiser, to expand the program, they are unable to provide coverage outside their service area at this time.

If you are a farmer in one of Kaiser’s 20 metro Atlanta service area counties, you may qualify for the Community Health Coverage Program. You can learn about the coverage area and other eligibility requirements by filling out the interest form below.

We are committed to creating a system where farmers are not at risk of losing their businesses or livelihoods due to unexpected medical expenses. In the meantime, if you're facing a sudden medical hardship, we encourage you to apply for direct cash assistance through The Farmer Fund
